If you are eligible for the Humana Dual Eligible Special Needs Plan based on your dual eligibility status and have one of these chronic conditions: Alzheimer’s disease or dementia, arthritis, cancer, congestive heart failure, diabetes or HIV/AIDS; this plan will waive the premium when you use in-network providers.
